I read another story that explains the 'why' of transmission: that this is a blood-bourne disease getting treated in clinics and hospitals that re-use bedding, needles, gowns, gauze! That sanitation is a brief hand-washing, if that. Then when someone dies, the custom is for their relatives to immediately wash the body - the bloody, virally-hot body - and relatives are hiding their sick / dying / dead ... leading to even more spread of disease.
